Glaucoma is a group of eye diseases that progressively damage the optic nerve. Elevated intraocular pressure is an important and treatable risk factor, although glaucoma can also develop or progress when eye pressure is within the statistically normal range. Because early glaucoma frequently causes no noticeable symptoms, regular examinations and appropriate testing are essential.

Our glaucoma care is led by Deepan Selvadurai, M.D., a Mayo Clinic–trained, high-volume glaucoma and refractive cataract surgeon with extensive experience in both traditional glaucoma surgery and minimally invasive glaucoma surgery (MIGS). He was an early adopter of MIGS and has performed a large volume of procedures across multiple surgical platforms. His work extends beyond clinical use and includes clinical research, device development, early evaluation of new surgical technologies, and ongoing collaboration to refine existing procedures and develop future glaucoma treatments. This breadth of procedural experience allows him to tailor treatment according to the type and severity of glaucoma, previous procedures, and each patient’s individual needs and goals.

How Buffalo Ophthalmology Approaches Glaucoma Treatment

Our glaucoma treatments in Buffalo are focused on lowering eye pressure, improving fluid drainage, and slowing or halting vision loss.

Medicated Eye Drops

Often, the first step for early-stage glaucoma is medicated eye drops. These drops either reduce the eye’s fluid production or improve fluid drainage to lower pressure.

Laser Procedures

Laser treatments are used to enhance drainage and reduce IOP with minimal recovery time.

  • Selective Laser Trabeculoplasty (SLT): Stimulates the eye’s natural drainage system.

  • Laser Iridotomy: Creates a small opening in the iris to relieve pressure in angle-closure glaucoma.

Trabeculectomy

This surgical procedure creates a new drainage pathway by forming a small flap in the white of the eye (called the sclera). Fluid drains into a small reservoir under the conjunctiva, lowering pressure. In some cases, we may use an EX-PRESS® shunt to increase control and safety.

Shunts, Stents, and Valves

These microscopic devices are implanted to improve fluid drainage in more advanced cases. We use a wide range of options, including:

Minimally Invasive Glaucoma Surgery (MIGS)

Buffalo Ophthalmology was the first in Western New York to offer the OMNI® Surgical System. Dr. Selvadurai was among the first surgeons both nationally and in Western New York to offer the OMNI® Surgical System. OMNI combines canaloplasty, which viscodilates Schlemm’s canal, with trabeculotomy to improve access to the eye’s natural drainage system.

Dr. Selvadurai participated in the early clinical research and development of OMNI and continues to participate actively in the development and refinement of OMNI and other emerging glaucoma surgical technologies. He has extensive, high-volume experience performing OMNI and other MIGS procedures.

This drop-free treatment uses a biodegradable implant to deliver long-lasting medication directly to the eye. It offers sustained IOP control without the need for daily eye drops—an excellent option for patients with medication intolerance or adherence challenges.
